The University of North Carolina has had a state-sponsored medical training program since 1879. It is one of the most highly rated public programs in the United States with a 98 percent pass rate for first-time test takers on The United States Medical Licensing Examination Step 1 exam. The Office of the Chief Medical Examiner (OCME) in North Carolina offers a one-year fellowship in forensic pathology to qualified candidates. The OCME works with the UNC hospitals as well as the Department of Pathology and Laboratory Medicine. Fellowship recipients can see up to 250 autopsies a year, representing a wide spectrum of natural and traumatic causes of death.

The University of Texas Southwestern Medical is located in Dallas/Forth Worth. More than 2,400 residents and fellowships receive training at the school annually. In conjunction with Dallas County, UT Southwestern Medical School offers a forensic pathology fellowship that is a year of intensive activity designed to help develop the needed skills for a forensic pathologist. The fellowship offers students 250 to 330 autopsies per year, teaches them the expectations of making related courtroom testimonies and how to be actively involved with the supportive investigation division of the medical examiner's office. It also provides research and conference opportunities.

The University of Minnesota (UMN) is located in the twin cities of Minneapolis and St. Paul. Prospective fellowship students will find more than 920 medical students and more than 800 residents and fellows, along with 1,600 medical school faculty physicians and scientists. A forensics pathology fellowship at University of Minnesota gives the fellowship holder cases to work on under the supervision of a pathologist, encourages fellows to work on research projects and allows him supervise junior residents. Fellows also work up cases and elaborate preliminary diagnoses before staff pathologists review and discuss them.
