Open your current report document, and scroll down to the end of your references page. The first appendix will begin on a new page following the references.
Format the first appendix. The American Psychological Association's style tutorial indicates that you should label this addition "Appendix,", making sure it is centered and the text is double-spaced. Place the title of the appended material on the line below, and then insert the rest of the material.
If your appendix cannot be electronically added to your document--such as large documents, maps, signed consent forms, drawings or illustrations--NOVA Southeastern University indicates that you should include a cover page for each appendix that features "Appendix" centered on the page as well as the title or a description of the appendix on the next line.
Label your appendices for organization and clarity. If there is only one appendix, just label it "Appendix." However, if there are two or more appendices,distinguish between them by labeling them Appendix A and Appendix B. Appendices should always be labeled with uppercase, sequential letters.
Insert accurate page numbers for the appendices. Make sure to include the material that is not electronic, such as maps and consent forms. Manually insert the correct page numbers into the electronic document rather than using auto-generated page numbers.
