Create a section on the top of the report card for writing the student's name and the year. Include the school's name, the teacher's name or even a class slogan if you wish.
Organize the report card into several different grids with defined rows and columns. Label each grid with a section header such as behavior, motor skills, pre-reading skills, math readiness and social development.
List the pertinent skill sets in the rows beneath each section header. For example, under "social development" list "plays well with others" and "exhibits self-esteem." Under "math readiness" include the ability to sort shapes, count to ten, and recognize and create patterns.
Create columns for each grid indicating the student's readiness for kindergarten - such as excellent, moderate or needs improvement - to specify how well the student is progressing in each category. Avoid using red ink and conventional letter grades or percentages. Instead, provide more qualitative elaboration of strengths and weaknesses in a comments section on the report card.
Keep the tone of the report card informative and optimistic, as each student learns and develops at his own pace. Give parents an honest evaluation of their child's progress and offer actionable guidelines for improvement.
