      Gather all of your information directly related to your GED testing date such as the year taken and where you took it.

      Locate the state’s Department of Education GED Testing Program Office online or by phone. Find out what the state’s GED testing office’s requirements are to request a copy of your diploma. If online, download or print a copy of their request form, or if on the phone, request a copy of the form be sent to you.

      Gather your contact address and phone, your social security number and date of birth to help fill out the request form. Make sure to use the name you tested under.

      Complete the request form, sign and date it. If the GED office requires a payment, include a check or money order if mailing the request. Use a credit card if submitting the request online or by fax.
