  http://www.ged.ed.state.pa.us/GEDOnline/TranscriptRequestsOL/wfTranscriptRequest1.aspx.

  Enter all of the test takers information into the form located below "Examinee Information" after all information has been entered click the "next " button.

  Enter the address where you would like your GED transcript mailed. When complete, click the "next button".

  The transcript request from will be created automatically. Print the form. Sign and date at the bottom of the form.

  Mail the form along with a $3.00 money order made out to the "Commonwealth of PA" to the following address:
