Visit the Department of Education for the state where you took the GED test. If you have relocated since taking the test, go to the website for the state of your previous residence.
Click "Forms" from the website's menu. You may need to first click on "For Students" or "Adult Education" to reach this menu.
Download the GED Transcript and Diploma request form and print it out.
Check the box corresponding to "Diploma" or "Certificate" on the top of the form.
Fill in all of the information on the form. This will include your name, current address, Social Security number, date of birth, the date you took the test and the location of the testing facility.
Enter in the address where you want the diploma sent if it is different from your own.
Enclose a cashier's check or money order for the diploma. Fees vary by state, with New York, for instance, charging $10, while Indiana provides copies of your diploma at no charge.
Mail the form and payment to the address provided by the Department of Education.
