Type the number "1" in the upper right corner of the header. Pagination begins on the title page.
Place the running head, an abbreviated title typed in all uppercase letters, flush left at the top of the title page but below the manuscript header.
Choose a title that is a concise summary of the paper and stands alone as an explanation of the contents therein. The recommended length is 10 to 12 words and should not include abbreviations.
Position the title in the center of the top half of the page. Use both upper and lower case letters. If the title is more than one line, use a double space.
Center the authors' names, in order of their contributions; one double spaced line below the title. Use upper and lowercase letters and separate suffixes from their names, with a space instead of a comma. For two authors, insert the word "and" between the names. For three or more authors, include commas between each name and the word "and" before the last author listed.
Center the institutional affiliation one double spaced line below the authors' names. If there are several authors from different institutions, list each author on a separate line with their respective institutional affiliation typed one double spaced line below their name. If two of the authors are from the same institution, include them on the same line.
