Duke University School of Medicine manages the Surgical Physician Assistant Residency Program. While in the 12 month program, residents rotate through 16 surgical specialties ranging from cardiothoracic surgery to plastic and reconstructive surgery. Admission requires completion of a physician assistant program at any degree level (certificate, bachelor's or master's) and eligibility to the national physician assistant licensing exam if not already board certified. Applications are due in late winter, and residency starts in early September.

The Basset Healthcare Department of Surgery conducts the Multispecialty Surgical Physician Assistant Postgraduate Training Program. Residents complete ten mandatory and two elective surgical rotations over 12 months throughout Bassett's four regional hospitals. In addition to graduation from an accredited physician assistant program, applicants must have already passed the national licensing exam and must have a New York state physician assistant license or eligibility to apply for it. Residents matriculate in early September or early January.

The Postgraduate Surgical Residency for Physician Assistants at Johns Hopkins Hospital was the first surgical residency program established. Residents rotate through six surgical specialties over 12 months. Applicants must have at least a master's degree from a physician assistant program with a 3.4 GPA in addition to completion of the national licensing exam. Residents can start the program in either April or October.

The Surgical Physician Assistant Residency Program at Norwalk Hospital requires at minimum a bachelor's degree in physician assistant studies. The didactic portion of the program takes place at Yale University School of Medicine, and residents rotate through general surgery and nine other subspecialties at Norwalk Hospital over 12 months. Applicants must be eligible to take the national licensing exam at time of matriculation, which takes place in September.
