The School of Medicine at the University of Utah has its own department of internal medicine, which offers several areas of specialty, including nephrology, oncology, rheumatology, endocrinology, gerontology and cardiology. The school has a large research department and a physician scientist training program which performs post-doctoral research along with clinical medicine. The school also offers fellowships in pulmonary disease, infectious disease, gastroenterology and cardiovascular disease. Internal medicine residencies are served at the Salt Lake City VA Medical Center, the LDS Hospital and the University Hospital.

The University of Pittsburgh's internal medicine residency program has several tracks for aspiring internists. Doctors can take the general internal medicine track or enroll in more specialized areas, such as women's health, geriatrics or global health. There is also a clinical scientist training program available. Internists interested in women's health perform their residencies at Magee-Women's Hospital of UPMC (University of Pittsburgh Medical Center) while other internists go on to UPMC Shadyside Hospital, VA Pittsburgh Healthcare System, UPMC Presbyterian or UPMC Montefiore Hospital.

Wayne State University in Michigan offers internal medicine fellowships in endocrinology, cardiology, gastroenterology, geriatrics, rheumatology, oncology, hematology, infectious diseases, pulmonary care and general medicine. Residency sites are Detroit Receiving Hospital, the Detroit Medical Center and the Veteran's Administration Hospital, where internists rotate through general wards, emergency rooms and sub-specialty units, such as intensive care, critical care and coronary care. Senior internal medicine residents are also expected to provide teaching and mentoring to junior residents.
