How to Get the Official Copy of a STAR Student Report for SFUSD

The San Francisco Unified School District (SFUSD) provides an education to more than 56,000 students and reports the highest test scores of any large urban school district in California, according to the school district's website. SFUSD schools include early education, elementary, middle and high schools as well as charter schools. If you attend a school in the SFUSD, you can obtain a copy of your STAR Student Report by requesting your transcript.

Instructions

      Visit the SFUSD website, SFUSD.edu. Click "Go," which is under the "Get Transcripts" heading in a box labeled "How Do I?"

      Click "Transcript request form" under "get transcripts?" to download the PDF form.

      Complete the request for student transcript form, which authorizes the school in the SFUSD to release your scholastic record. You must provide the date, your personal information, current address, contact information and the number of copies you are requesting. There is a $5 charge for each copy (cost as of 2011).

      Complete your mailing address if you want the school to send the transcript to you by postal service. If you are taking the form to the SFUSD's transcript office, leave this information blank. Sign the request form.

      Write a check or money order for $5 and mail the letter to:

      You can also visit the office in person. Hours of operation are Monday through Friday, 8 a.m. to 4 p.m.
