Open Microsoft Word on your computer and type on the top of the page the quarter information. For example, "First Quarterly Report 2008-2009 School Year." Below that put the date and center those two lines.
Include “Student Information" in your quarterly report. This will include the student’s name, address, telephone number, date of birth, age, school district, grade, instructor’s name, curriculum that was used and the number of hours of instruction during the quarter.
Include academics. What has the child completed in this past quarter? I would include the subject, the chapters completed, a syllabi of what was completed and finally a grade--either a number, a letter, or just pass or fail.
Show extracurricular activities in the next section of the quarterly report. This would include things like physical education, music, scholarships, concerts, work experience, community service, field trips, boy scout activities, reading lists, and whatever other activity that your child has achieved in that quarter. Not only does a thorough quarterly report give you a great record for you child but it also lets your child “shine” in the eyes of the superintendent.
