Fill out the general information section of the form. On the New York GED form, this information is located at the top of the form and includes your Social Security number, preparation program number, name, address, phone number and email address as well as your gender, age and language proficiency.
Move to the previous testing section. Here, you will be asked if you have taken the test before. You will be required to list the location, the date and the identification number used for the test and your score. This information can be obtained at the place where you last took the test.
Proceed to the current testing section. This section will ask you where you intend to take the test next. You must indicate why you are taking the test by choosing one of several reasons offered or you can check the "other" box if your reason is not listed.
Progress to the eligibility section. In this section you must answer questions about your eligibility. If you are under 19, you must check the box that suits your situation. These boxes will present different scenarios which indicate when you left high school and why. If there is no box to match your situation, check the "other" box and offer an explanation in the space provided.
Sign your name. Agree to the terms of the form representing your release for giving out scores to testing centers. Sign the form where it says "candidate signature" and include today's date. If you are under 18 years of age, you will be required to have your parent or guardian sign the form on the bottom.
