Sport helmets for football and hockey have padding inside the hard shell that absorbs the kinetic energy from hard hits. This padding protects the skull and facial bones, including the cheekbones. When the padding wears out or becomes stinky from sweat, replace it with a new and custom-cut pad. Adding a homemade cheek piece to the helmet is a matter of cutting an appropriate sized chunk of closed-cell foam and attaching it to the old cheek piece's location.
