How to Make a QA Cover Page in MLA Format

Among the style formats for scholarly publications, reports, student essays and papers, the Modern Language Association, or MLA, style is most often used in the humanities field. MLA style includes the use of short, in-text citations, 1-inch margins, double-spaced lines and 12-point font. A quality assurance, or QA, report is typically written on a quarterly or annual basis and may include an abstract that summarizes the report, background information, findings and resolutions. Though MLA does not use a separate cover page, you can use title page guidelines to format your QA report cover page.

      Set your word processor to 1-inch margins on all sides and double-space all text. Left-justify your margin, so that all the words align on the left side of the page.

      Type your name, the name of your company, your work division, your job title and the date of the QA report in the upper left corner, which is an inch from the top of the page.

      Double space and select "Center" on your word processing program. Type the title of your QA report in standard capitalization rather than using all uppercase letters. For example: "Quality Assurance Report for the Second Quarter."
