List the name of the organization or website at which you located the information, followed by a period.
List the name of the index in which you found the entry, followed by a colon.
List the type of resource, followed by a comma. When citing vital records, the type of resource will be either a birth record entry or a death record entry.
List the volume number, if applicable, followed by a comma.
List the page number, followed by a semi-colon.
List the URL of the web page on which you located the entry, followed by the date that you accessed the information.
Make sure that the citation appears in the following form:
Name of Organization or Website. Name of Index or Database: Type of Resource, Volume Number, Page Number; viewed online at http://www.website.com on today's date.
Here is an example of a correct citation:
