      Download the Michigan GED Transcript Request Form from the Michigan Department of Labor and Economic Growth website. Print the form and fill out all the fields. You will need to provide your name, date of birth, Social Security number, and the month and date that you took the exam. You will also be asked to check a reason for why you are requesting the transcript.
