Label the spine of a three-ring binder with the student's name and profile creation date. The information inside will change over time, but the date is there to remind you, and tell future teachers, when the file was originally organized.
Insert six dividers into the binder and label the tabs as follows: Personal Information, Educational Inventories, Testing Results, Recent Grades, Student Work and Teacher Comments.
Enter the student's personal information, such as full name, birth date, parent and guardian names, address and home phone number. If your school requires teachers to keep a copy of emergency contacts, place that information in this section as well.
Ask the student and his parents or guardians to complete several different inventories to determine the student's learning style, interests and learning intelligence. Completed by the student, a learning style inventory determines how the student learns best. An interest inventory should be completed by the student and his parents, listing things that the student likes in and out of school. A learning intelligence inventory taken by the student is used to categorize the way the student thinks, such as naturally, kinesthetically or logically. These inventories offer insight into how to best approach instruction with the student.
Place scores for applicable testing, such as DIBELS, state standardized tests and reading level assessments, in the "Testing Results" section. Update these scores whenever new testing occurs so that a teacher can refer to the student profile at any time for the most current information.
Insert a copy of the student's most recent grades. Include all subject areas.
Save samples of the student's work. Be selective in the assignments placed in this section; you can't include everything. Choose a few samples from each core subject to show progress and setbacks throughout the year.
Record and date positive and negative remarks about the student's performance in the "Teacher Comments" segment. These comments help with end-of-year assessments.
Pass the profile to the student's next teacher at the end of the school year. It becomes that teacher's responsibility to update and add to the profile.
