Complete the individualized basic reading inventory. Depending on your school and your needs, this typically includes a series of tests that are given to the students -- both orally and in writing. You will test their reading level, comprehension, vocabulary and general aptitude for reading. Your school might have an individualized basic reading inventory you are required to give, or you may design your own.
Tally the individual elements of the reading inventory, so that you have a picture of the student's reading ability.
Write a general reading statement that applies to the student, based on the inventory. For example, you might say, "Johnny reads at a fourth-grade level, but has the spoken vocabulary of an eighth-grader." You might also include statements like, "Johnny performs at grade level, but he has problems with spelling."
Expand your summary to include any additional information that your individualized assessment discovered about the particular student.
