How to Fill Out a Blue Card for Boy Scouts

The Blue Card, also known as the Merit Badge Application, is a document that indicates a scout's progress toward completion of a merit badge. First, the scoutmaster must complete and sign off on the Blue Card. He will then forward the card to the counselor, who will then complete his portion of the scout's Blue Card. Once the card is complete, the card is then forwarded back to the scoutmaster, who will then bestow the appropriate merit badge upon the scout.

Instructions

  1. Scoutmaster

      Enter the name of the scout, his address, scout level -- boy scout, varsity scout or venturer -- the type of unit to which he belongs -- troop, team, crew or ship -- the unit number and the name of the unit's district and council on the front of the Blue Card before the beginning of the merit class. Make sure to also sign and date the Blue Card.

      Enter the type of merit badge (e.g., Rabbit Raising) on the first page on the back of the card.

      Include the scout's name in the name field in "Applicant's Record" on the back of the card. Also indicate the type of merit badge in the appropriate field again underneath the scout's name.

      Enter the applicant's name again in the applicant field in the "Counselor's Record" section on the back of the card. Check the group type -- troop, team or crew -- and enter the unit number. Again, enter the merit badge type on this page.

      Submit the card to the counselor before the merit class.

    Counselor

      Enter your name (the counselor), address and telephone number in the "Counselor's Record" portion of the card.

      List each individual requirement including any subparts, the date the requirement was completed, and your (the counselor's) initials in each field on the front of the Blue Card.

      Sign the "Signature of Counselor" fields in the Applicant's Record on the back of the card. Insert the date the merit badge was earned next to the signature fields and in the "Counselor's Record" section on the back of the card.

      Tear off the "Counselor's Records" section of the Blue Card to retain for your records. Return the remainder of the Blue Card to the Merit Badge University, which will then forward it to the scoutmaster.
