Enter the school's information at the top of the sheet. Include the name of the institution, the address, the telephone or fax number, the website and the e-mail address. If you have a picture of your school's emblem on your hard drive, copy it on the document file and adjust its size.
Write the title of the document -- "Student Information Form" -- in large letters under the school's details. Use a large font size for this line, at least six points bigger than the size of the rest of the document's fonts.
Leave at least two rows of space and enter "Registration Number" on the left side of the page, using bold letters. As this is an official school document, avoid calligraphic fonts and use simple ones, such as Arial, Trebuchet or Times New Roman.
Press "Enter" twice, leaving a row of space after the previous line. Write "Student's name" in bold letters and hit "Enter" twice to move on to the next part of the form. Repeat the process to create lines for the student's address, grade attending, class, home phone, birth date and -- for colleges -- the high school attended.
Leave two rows of space and write "Guardian's name." Enter lines for the guardian's contact number -- mobile or work phone -- and e-mail address. Repeat the process for the second parent or guardian, if applicable.
Enter the sentence "I confirm the validity of the above information" or a similar declaration, near the bottom-right corner of the sheet. This is where the guardian must sign after he has reviewed the information sheet.
