Open your new blank spreadsheet. On the first row, center your cursor and type in a title, such as "Class Evaluation" or "Education 4410 Spring Evaluation." Make sure the title identifies the specific class, including semester, year and any assigned code numbers. Skip two rows and start with the evaluation criteria.
Create a table chart that includes basic labels such as, instructor, teaching effectiveness, technology, real-life examples, lesson plans, discipline management, classroom materials and environment. Put each label on an individual row on your spreadsheet. All your items should be organized vertically.
Create columns that will evaluate the effectiveness of each criteria. Consider create the columns that rank by effectiveness. Type in very effective, somewhat effective, neutral, no difference, fair and poor.
