University of the Southwest offers replacement diplomas to alumni for two reasons. Alumni may request a new diploma displaying a new name. For example, a graduate can choose to reprint a diploma displaying a married name. The university will also reprint diplomas for students who enrolled under the name College of the Southwest, but graduated under the name University of the Southwest.

      Request a Diploma Request Form from the Office of the Registrar. You can download this form from the website, or call the university and request one by mail or fax.

      Complete the request form.

      Check the appropriate box, choosing the reason a replacement is needed. You can choose to update your name, or have your degree printed with the university's old name.

      Mail your reprint fee and form to the address on the application. Your reprint fee will depend on the reason you are requesting a reprint.
