Download the Mid-Year Report file and Secondary School Final Report file found at the Common Application "Downloads" page. You will need to send the Mid-Year Report document, along with your 12th grade first semester transcript, to update Stanford on your senior year academic status and changes to extracurricular or character record. The Secondary School Final Report should be submitted by your counselor after final grades have been submitted.
Complete the "To the Applicant" section of the Mid-Year Report to be submitted at the end of the first semester of your senior year. You will need to give the document to your counselor to fill out the rest of the section under "To the Secondary School Counselor." The counselor will need to attach an official transcript, school profile and transcript legend. The transcript should have all the courses that you are currently taking as well as courses you have already passed in your high school academic career.
Fill out the Secondary School Final Report after final grades have been posted at the end of the school year and request your counselor to attach an official high school transcript, school profile and transcript legend. You must fill out the top section of the form titled "To the Applicant," while the counselor fills out the remaining portion under the "To the Secondary School Counselor" section.
