Separate the title page from the annotated bibliography, making sure they are on different pages. The title page will go before the annotated bibliography when submitted.
Set your font to Times New Roman, 12 point, unless instructed otherwise.
Insert a running header on your title page. Place the header in the flush upper right hand corner of each page and include the title of your paper and the page number. Use the "insert" option in your word processing program to add running heads.
Add three double spaces from the upper margin of the title page to the title itself. This is where your title page will begin and also where you will add the title of your paper. Make sure it is centered between the right and left margins of the paper.
Click down four double spaces from the title to add your student information, also centered. Type your full name, the name of your college, the name of the program that you are in, and the professor's name. Separate each piece of information by one double space.
Go down two more double spaces from the last entry and type in the date that you will submit the assignment. You now have a title page for your annotated bibliography.
