Draw a full circle in the center of the right-hand side of your index card. Trace the quarter with a marker to form a full circle and leave the inside of the circle blank, with only the outline surrounding it. Label this circle as the new moon and also make a note on the card identifying it as "Day 1."
Draw circles by tracing the quarter onto the other 27 index cards in the same position as the first, labeling each card consecutively as "Day 2," "Day 3," "Day 4," etc.
Watch the sky every night or check a lunar calendar and draw the pattern of the moon, beginning one day after the new moon, onto the appropriate numbered index card, filling in the shape of the moon each day inside the circles you drew in Step 2. Repeat for each of the 28 days of the cycle.
Label the New Moon (Day 1), Waxing Crescent (Day 4), First Quarter (Day 8), Waxing Gibbous (Day 12), Full Moon (Day 16), Waning Gibbous (Day 20), Last Quarter (Day 24), Waning Crescent (Day 28). Write these different phases under the image you drew onto the circle to identify the same of the moon as well as its stage.
Stack the cards in order and staple them together on the left side. Hold the cards with your left hand over the staple and flip through the cards with your right hand to display the phases in an animated fashion.
