Obtain forms. If your child's school does not give you an enrollment form on the first day of school, ask the office for one. It is self explanatory and easy to fill out.
List names of children. One form will be able to enroll your entire family into the program if all of your children attend the same school. Depending on the policies of the school district, you may be required to fill out a form for each school (for example, if you had a child in elementary and another in junior high). It will require you to list the names of your children and their grade level.
Know your household income. The household income is the total income that is brought into your household. You will need each adult's name, gross income and how often they get paid.
Learn results. It is easy for you to be able to figure out if your family will qualify for free or reduced lunches, or if they do not qualify at all. On the form, income guidelines are listed in chart form and changed from year to year. Even though you will be able to know your qualifications at the time that you fill out the form, it is the responsibility of the school officials to officially confirm the results of your qualification.
