Examine the student's record and special education plan (Individualized Education Plan, if one exists) to determine the extent of the disability and what measures have been taken up until this point for inclusion of education. Take note of things such as the prior educational environment of the student, as well as any accommodations that have been made for the student to be educated in any environment.
Make any changes in your classroom environment so that you will meet these accommodations. Examples include installing a wheelchair ramp, handicap accessible bathrooms or widening your lesson areas.
Create a plan that gradually acclimates the student to the new environment. For instance, begin by having the student come to your classroom for an hour a day for the first couple of days, and then two hours a day. Gradually ease the student into the full routine of your classroom.
Make changes to your transition plan as you actually transition the student, because each individual student, and each situation, is different and requires a different level of accommodation.
