Create a chart in your word processing program that has three columns and one row for each week in your semester, plus an additional row for the column labels. Label the left column "Week Ending" and fill the rows with the dates of consecutive Fridays. Label the center column "Signature" and the right column "Notes." At the top of the page, type a short message asking parents to review the student's recent assignments and sign the center column.
Print one copy of the chart for each student. Tape the chart securely to the inside cover of a binder and label the spine of the binder neatly with the student's name.
Create simple graph templates using your word processing software by creating a chart with 10 rows and 10 to 20 columns. Select each row and define the borders so that there are no borders except a dotted line as the top border. Select the whole chart and define solid left and bottom borders to create the axes on the graph template. Use text boxes to label the y-axis in 10 percent increments. Print one copy per assessment area per student and place them in the students' data folders.
Instruct students to write their names on their graphs and to label each graph with a title, such as "Spelling Tests," "Respectful Behavior" or "Attendance." Define regular intervals for self-assessment, such as Friday afternoons or after every test. During self-assessments, let students graph their progress in a given area according to a well-defined rubric. Keep the graphs in the students' data folders when not in use.
At the end of each week, select a few representative assignments and place each student's assignments into her folder. Assign students to take the folder home, discuss the contents with their parents, ask their parents to sign the chart for the week, and bring the folder back on Monday.
