Print or type the calendar year, from January 1 to December 31, at the top of the first page of your Tier I report. Include page numbers in "Page Number of Total Pages" format if your report goes beyond one page.
Identify the facility about which you are reporting. Give the complete name and company identifier of the facility, such as "Bel Gioioso, Freedom Plant." Provide the full street address, including city, state and zip code if available. Include the latitude and longitude if street address is not available.
Enter the North American Industry Classification System (NAICS) code as well as the Dun and Bradstreet number for your facility. Contact the state or regional office of Dun and Bradstreet if you cannot locate this information.
Provide the contact information for the owner or operator of the facility. Include an emergency contact that response agencies can refer to if unable to contact the owner or operator. Ensure that emergency response agencies can obtain emergency information at all times, such as through an automated information phone line.
Follow the Environmental Protection Agency (EPA) guidelines for categorizing the descriptions, amounts and locations of physical and health hazards. Indicate amounts of hazardous materials as weight in pounds and provide the general locations where they are stored. Calculate both the peak amount of hazardous materials and the average daily amounts. Follow EPA guidelines for calculation methods.
Enter next to each hazard category the greatest number of days that a chemical from the category was on-site.
Certify that all information entered in the Tier I report is true, accurate and complete. Enter your full name and title. Sign and date the form. Include the total number of pages, including attachments such as material safety data sheets or facility maps, in your submission.
