Describe the student's behavior during testing including attention, cooperation and any unusual behaviors.
List all tests that were administered.
Provide test results organized into categories of assessment type, such as intellectual functioning, academic achievement, and emotional factors. Describe the student's behavior during testing, especially those behaviors that may suggest deficits in understanding, processing or behavioral control. Include information on what the each test is measuring to facilitate interpretation of the significance of the results.
Provide professional insight into how to interpret the test results. Compare the student's performance with expectations of same-age peers and identify significant discrepancies in results across different areas, such as input types, domain specific cognitive processing and output.
Summarize your findings and impressions. Make recommendations on the most appropriate educational setting and strategies as well as any ancillary services that may benefit the student such as counseling or tutoring. Use terminology that can be understood by teachers, counselors, and parents.
