How to Write a Testing Report

Standardized testing is an important part of educational assessment. Achievement or ability tests can help pinpoint academic strengths and weaknesses. This enables education professionals to adapt instruction to students' individual needs. The evaluation report is an important tool in conveying information gathered from the assessment process. The report must communicate testing conditions and findings clearly and concisely so professionals and parents can utilize the data to make informed educational decisions.

      Type "Evaluation Report" at the top center of the page. Below this, type the name of the facility, company or school associated with the assessment.

      Skip a line and type the following headings for student-identifying information: "Full Name," "Current Address," "Date of Birth," "Age", "Race," "Gender," "Parent or Guardian," "Attending School" and "Grade Placement". Use a new line for each heading.

      Locate the student's demographic information on the student's permanent record.

      Type student-identifying information into previously typed headings.

      Type the examiner's name and date of the report.

      Type a section heading "Purpose of Assessment."

      Describe briefly the reason for the assessment. Include a description of any concerns about the student's current performance and intervention strategies that have been attempted. This section should be in narrative rather than a list format.

      Type a section heading "Background Information."

      Communicate known background information related to the student's current performance. This may include medical, social, family, legal or mental health information. This information should be arranged in chronological order. Sensitive information should be corroborated with the student's records.

      Type a section heading "Procedures Administered."

      List the names of all tests or test components given to the student that will comprise the body of the report.

      Type a section heading "Behavioral Observations During Evaluation."

      Describe the student's attitude, interaction and approaches to the test situation. Report any unusual conditions or behaviors in this section. Explain the environment in which the assessment was conducted (quiet room, busy hallway, etc.).

      Type a section heading "Results of Evaluation."

      List each test given and the results obtained from that test.

      Type a section heading "Discussion."

      Explain in narrative format what these test scores mean. Include information comparing ability and achievement scores, expected versus actual achievement or grade level expectations versus the student's performance.

      Type the section heading "Summary and Recommendations."

      Relate in narrative format how the test results confirm or contradict the hypotheses stated in the Purpose of Assessment section. Include recommendations for further assessment, educational placement or intervention procedures.

      Type the name of the person completing the report at the end of the report. List the person's degrees, job title and certifications. If another individual typed the report, his initials (in lower case letters) should be typed beneath the examiner's credentials.
