Download the family law forms from your local or county family law court website. Purchase the preprinted form packet from the self-help center at the family court filing counter in your county if you prefer to manually fill in the papers.
Write your legal name, current address and telephone number and the name of your county at the top caption of the first page in each form. Indicate the address where you prefer to receive your mail from the court if you do not want to give out your house address. Write the name of your spouse, child or any other party involved in the case.
Fill out the case number in the lower right caption box of every page of the form. Ask the court clerk for the case number if you do not have it or if it is the first time you are filling in the forms.
Check the type of family case that you are engaged in, such as divorce, child custody, visitation, or juvenile incarceration. Indicate that something is not applicable to your case by writing "N/A." Write "in pro per" on the "Attorney for" space to indicate that you do not have a lawyer. Write your attorney's name if you have one.
Sign at the bottom page of the form in the presence of a notary public, such as an attorney or the court clerk. If you are serving another party, indicate the other party's name, address and the means of service on the attached certificate of service after the party has been served and before filing the forms with the court clerk.
