Access the "ADA: American Dental Association Testing" page on the Prometric website. The company has a list of Kentucky DAT test locations in an online database. Click the "Start" button to begin accessing the database.
Choose to look for Dental Admission Test locations in Kentucky using the menu options. When you finish setting your options, the system will display a list of current Kentucky DAT test sites. You can also get directions to each Kentucky DAT test location. As of October 2011, cities with DAT testing centers in Kentucky include Louisville, Lexington and Florence.
Obtain a DENTPIN number from the American Dental Association. The ADA does not allow you to apply to take the DAT test until you obtain this number. You may register for a number on the ADA's "DENTPIN Registration" page.
Fill out the DAT application. The ADA provides an online application on its "DAT Application" page. Alternatively, you may make a written request for a paper form using the address below. You may also send a fax request to 312-587-4105. As of October 2011, the fee is $320 to take the exam. You may pay by credit card, money order or certified check. Make checks out to the "American Dental Association."

Wait for an eligibility letter from the ADA. When you receive this letter, you can then register with Prometric to take the DAT at your chosen Kentucky test site. From Prometric's "ADA: American Dental Association Testing" page, click the "Start" button. On the next page, select "Kentucky" as the location. Select "Schedule a Test" on the next page to schedule your Dental Admission Test. You may also schedule by calling 800-688-5804.
