Fill out the package of materials that the college will send you and read all the information. Make sure to sign all of the consent forms.
Save a copy of the consent forms and information with your next-of-kin. Also store copies of all of these materials with your own personal documents and will. You may revoke your consent at any time by writing to the medical school.
Specify instructions about the future plans for your body in your will.
Inform your next-of-kin about your will and your plans for your body. Also inform another person in case of any unforeseen circumstance with your next-of-kin. Tell your next-of-kin to inform the Medical College immediately after your death to confirm acceptance.
Arrange for the Medical School of Wisconsin to be notified when you die. Specify these instructions in your will and also inform your next-of-kin of this duty. Your body will be transported by the Medical School of Wisconsin until it is needed.
Carry the donor card sent to you by the Medical College of Wisconsin at all times.
Make arrangements for transportation of your body.
Make other arrangements for your body in the case that it is rejected upon death. The Medical College of Wisconsin accepts bodies of all ages, but certain diseases or unforeseen damages to your body can preclude acceptance. Discuss these with the Medical College but also make alternate arrangements in the unfortunate case that your body is not accepted upon death.
