Write the name of the author of the memoir on your index card with the last name first, followed by a comma, the first name and a period at the end. For example if the author of the memoir is Joe Smith, you will write the name as Smith, Joe.
Record the title of the memoir after the author's name. The title should be italicized and written in title case, which means you capitalize all words in the title except articles ("a," "an" or "the"), short prepositions (such as "of" or "for"), coordinating conjunctions ("and," "but" or "so") and the word "to" when used in infinitives ("to eat" and "to see"). Place a period at the end of the memoir's title. For example:
Smith, Joe. How I Learned to Cook.
Include the place of publication next with a colon after it. Follow the colon with the book's publisher, a comma, the year of publication and a period. For example:
Smith, Joe. How I Learned to Cook. New York, New York: Morning Star Press, 1999.
List the medium of publication. For a printed book you will write "Print." Follow the medium with a period. For example:
Smith, Joe. How I Learned to Cook. New York, New York: Morning Star Press, 1999. Print.
Write the last name of the author of the memoir first, followed by a comma and the first initial. For example:
Smith, J.
Place the year of publication in parentheses after the author's name, followed by a period. For example:
Smith, J. (1999).
Include the title of the memoir, followed by a period. Italicize the title (and the subtitle if there is one), but capitalize only the first word in the title and any proper nouns. For example:
Smith, J. (1999). How I learned to cook.
Write the location in which the memoir was published, a colon and the publisher, followed by a period. For example:
Smith, J. (1999). How I learned to cook. New York, New York: Morning Star Press.
