      Go to the Federal Student Aid Homepage (studentaid.ed.gov) and click on "Repayment Information." There is a long list of options, so scroll down the screen until you see the section labeled "Cancellation and Deferment Options for Teachers." Click on that link. If you received this application from your lender, skip down to step 5.

      Click on the link that says "Stafford Loan Forgiveness for Teachers." This should take you to the next screen which states the eligibility requirements and provides the form you need.

      Determine if you meet the eligibility requirements, which include having been employed as a full-time teacher for five consecutive years in an elementary or secondary school that serves low-income families, and in a critical-need area such as special education.

      Scroll down to the section labeled "How to Apply" and click on the link that allows you to print a copy of the Teacher Loan Forgiveness Application.

      Fill out the necessary information in Section 1, which includes your social security number. Sign your name in the section that asks for your signature.

      Check the box that applies to you in Section 2, which helps determine your eligibility. Sign your name where it asks for the borrower's signature.

      Have your Chief Administrative Officer, such as your principal, assistant principal or superintendent; complete the information in Section 3.

      Mail the application to the address provided in Section 9. If the address is missing, contact your loan provider.
