How to Create an Open-Ended Questionnaire for Evaluations

One of the key tools used by an organization is the continual evaluation of current processes and business practices. Evaluation helps you realize what works and what does not work in the process or business practice you have created. While most questionnaires feature ratings or Likert scale questions, creating an open-ended questionnaire is beneficial in gathering detailed feedback about your organization. Use the feedback in order to fine tune your delivery the next time around.

      Create directions. The top of the questionnaire should have a brief statement about the purpose of the questionnaire. This will give those who will ultimately fill it out an understanding of what you hope to achieve. This will motivate them to answer questions openly and honestly. Make every effort to keep the answers to the questionnaire anonymous.

      Draft the questions. You have full control over the questions, so draft them with the goal of attaining the most pertinent information possible. As such, try to keep the questions focused on certain aspects of your program. For example, a question could read, "What worked best within the lesson on Shakespeare?"

      Draft the final questions. You have already asked specific questions of your choosing. At the end, you should draft a final overarching question. This will allow the respondent to state anything they wish to cover that wasn't already covered in the specific question. For example, this could read, "Any other comments?"
