Hawkfrost doesn't die in a single specific book. He dies in "The Broken Code" (specifically, in the third book of the arc, "The Broken Code: Darkness Within").
He is killed by Firestar and Brambleclaw in a battle where he attempts to unleash a powerful dark force.
