Psychological Evaluations Checklist

Psychological evaluations checklists measure specific behaviors or symptoms that may indicate a disorder. Checklists are questionnaires used by teachers, parents, or psychologists to determine if your child needs further evaluation for a psychological disorder, and as such do not diagnose disorders but may indicate symptoms. The disorders measured may include behavior, attention deficit hyperactivity, depression and learning disorders. Your child may be demonstrating certain behaviors that flag psychological problems. A rating scale grades the questionnaires from no problems indicated to severity.
  1. Conner's Rating Scales Revised

    • The Conner's Rating Scales-Revised, (CRS-R) is an instrument or checklist for parents, teachers and psychologists to rate and determine Attention Deficit Disorder (ADD) or Attention Deficit Hyperactivity (ADHD) characteristics. The symptoms include consistent inattentive behavior in the classroom, inability to complete tasks, difficulty sitting still, demonstrating compulsivity, quickly moving from one task to another without completion, yelling out answers in the classroom and difficulty developing relationships with peers. If your child demonstrates symptoms that disrupt the classroom or if he is highly over-active as compared to his peers, he may have ADD, according to LDonline. Parents and teachers complete the (CRS-R) checklist for certain behaviors for the school psychologist to determine if further evaluation and a medical diagnosis are necessary. The scale measures behaviors from no indicated problems to severe problems, according to Conner's Rating Scales.

    Children's Depression Rating Scale

    • The Children's Depression Rating Scale (CDRS) is a questionnaire use to determine depression in children and its severity. Assessment information is gathered from the parent, child and teacher by completing the checklist. This scale measures mood affect demonstrated by your child. Other behaviors that are measured are self-esteem, social withdrawal, irritability and working capacity in school. Sixteen behaviors are measured to determine your child's severity of depression and medical attention needed, according to Lifewise on line.

    Cognitive Processing Rating Scale

    • The Cognitive Processing Rating Scale (CPI) is a scale for determining if your child has a learning disability in one of the psychological areas. These areas include reading, math, language, writing, auditory and visual processing. The rating scale will measure or indicate a learning disability present by how your child is performing in a certain subject area. Parents, teachers and students determine information processing that may indicate a learning disability by completing the CPI checklist. An auditory processing disorder is a deficit in sorting out of sounds and words correctly while listening to spoken language. A visual processing disorder displays difficulty discriminating between letters, shapes, colors and objects. Math, reading and writing learning disabilities show delayed performance as compared to age and grade levels of expectancy, according to ldinfo on line.
