How to Request a Re-evaluation of an IEP

An individualized education program, or IEP, is offered by public school districts to help students with disabilities and learning challenges do well in school. The school evaluates a student for an IEP and determines the special services for which that student qualifies. However, as a student progresses, an IEP may become outdated and parents need to request a re-evaluation to keep services updated.

      Contact your child's school district to obtain the required form for requesting a re-evaluation. Some schools offer the request form online, while others may need to mail you a copy.

      Fill out the form. In addition to identifying information, the form requests a reason for the re-evaluation and any changes in your child's diagnoses.

      Submit the form to the school district for consideration. The school examines the reasons and changes to determine if your child qualifies for a re-evaluation. If a re-evaluation is deemed necessary, the evaluation process is reinstated. Many school districts finish evaluations within 90 days.
