How to Set Up a 5-Point Survey

Surveys are useful for measuring the opinions of a group of people. Workplaces, schools and stores are a few examples of organizations that frequently use surveys. One common type of survey employs the use of a five-point scale. This type of measuring device is known as a Likert scale. You can tailor a Likert scale survey for your specific needs in order to enable you to obtain and analyze data that is important to you.

      Write a list of questions that measure people's opinions about a particular product or experience. For example, you might write questions pertaining to employees' opinions concerning a new work schedule.

      Add the numbers 1 through 5 above or below each question in the survey.

      Include an explanation of the scoring system at the beginning of the survey. Equate the numbers to the following labels: 1 = strongly disagree, 2 = disagree, 3 = undecided, 4 = agree, 5 = strongly agree.

      Analyze your data once the survey participants have turned in their results. For example, you might look at a particular question, such as, "I feel that the new work schedule allows me enough time for breaks during the day." If you surveyed 20 employees, you can average their responses to get an overall feel for the popular opinion concerning that question. To average responses, add the numbers together for each response and divide the total by the number of responses.
