Fill out the Chicago Public Schools Statement of Assurance. This document states your intention to remove a child from the public school system and lists the course materials that will be used for the process.
Write a letter for CPS describing the manner in which you plan to instruct your child at home. This letter should include previous experience, techniques you plan to use, and reasons for home schooling.
Fill out a Data Analysis & Progress Report form. This form lists all children you plan to home school, their ages, previous schools, subjects you plan to teach, and course materials you plan to use.
Mail the Statement of Assurance, Data Analysis & Progress Report and letter to the Chicago Public Schools at:

Mail a copy of the Data Analysis & Progress Report form to the Illinois State Board of Education.

File these forms annually to ensure continued permission by Chicago Public Schools and the Illinois Board of Education to continue homeschooling without penalty. Students who are kept home for schooling but whose parents do not fulfill the proper requirements will be considered truant.
