Write clear learning objectives for the training, including assessment and documentation of necessary skills and knowledge. Clear learning objectives explain the new behavior the trainee will perform on the job. Record a minimum of five learning objectives and a maximum of ten.
Construct an opening section of the evaluation form on a spreadsheet or in a table. Include general information like the name of the trainer, the name of the class and the date. Include a note to participants that the form will ask them to rate their skills and knowledge.
Transpose the statements from the learning objectives to the second section of the evaluation form's spreadsheet or table. Begin the first part of each statement with, "I have the skills/knowledge to..." and finish the statement with one of the learning objectives.
Insert a five-point Likert scale instrument into the spreadsheet or table directly below each statement. A Likert scale measures the trainee's positive or negative responses to each statement ranging from 1 for "No Skills/Knowledge" to 5 for "Outstanding Skills/Knowledge." Incorporate a check box below each answer.
Insert open-ended questions regarding the training experience into the spreadsheet or table at the end of section two to elicit written responses other than "yes" or "no." Leave adequate space below each question for answers and comments.
