Make sure all information you have is correct. Spell the teacher's name properly on any report you submit. Note the days and times of the class as well as the subject matter being taught.
Indicate what the teacher did well. A teacher should show up on time. Her lesson plans should be easy to understand and follow. All exams should be graded fairly. Class expectations such as homework and other written assignments should be spelled out the day the class starts. All textbooks necessary should be available for purchase in the bookstore, given out in class or easily ordered from another supplier.
Offer appropriate and precise criticism. If you found the assigned reading materials too hard, note this in your rating. If the teacher failed to handle disciplinary problems that resulted in frequent class disruptions, write about specific instances. Avoid any personalized and inappropriate critiques of the teacher such as disparaging descriptions of her dress or use of makeup.
Provide an overall summary and overall rating of the teacher's efficacy. The summary should indicate what you learned from the class as well as what you feel was left still unclear. Note the teacher's pedagogical methods, such as encouraging active listening and cooperative learning. If the classroom had a teacher's aide or an assistant teacher, note his role in any classroom activities.
