Before attending college, you must fill out and submit health and immunization records. Proof of immunizations for common illnesses such as MMR (measles, mumps, rubella), Tetanus-Diphtheria, Hepatitis B and chicken pox are required for all first-year students entering colleges, technical institutions and universities. Immunization forms are usually found on the university's website with information on deadlines and requirements. You must complete the form and have it certified by a health official to complete your application process.

Take the form to your health department or doctor's office. The immunizations form must be certified by a health official.
