How to Reference an Evaluation Form

If you are writing a research paper using a data set of evaluations, you need to know how to reference them properly in your paper. There are three general style guides that outline the format you should use to cite any reference: the American Psychological Association (APA) style, the Modern Language Association (MLA) style and the Chicago Manual of Style format. Each one has slightly different requirements for referencing data such as evaluation forms.

  1. Referencing an Evaluation Form in APA Style

      Reference the source of the evaluation form data or where the data is housed on the first line.

      List the name of the data file or evaluation form. Add the words "Retrieved from" followed by the website address where the data is located.

      Include information where the evaluation form(s) can be found if the data is not electronically housed.

    Referencing an Evaluation Form in MLA Style

      Reference the name of the evaluator followed by the words "Personal Evaluation" on the first line.

      Write the date the evaluation was done.

      Add the type of file at the end of the reference such as PDF file, Microsoft Word file or Digital file if the type of file is unknown.

    Referencing an Evaluation Form in Chicago Manual Style

      Number the evaluations in alphabetical order.

      List each evaluation by the last name of the evaluator.

      Follow the evaluator's name by a comma and the words, "evaluation form provided by the author."

      End with the date of the evaluation.
