Begin the letter by typing the date. Skip a space, and type the recipient's name, his department, and the department address. Skip an additional space, and type "Dear Dr./Ms./Mr. (Last name) followed by a colon.
Start the first paragraph by identifying the student under evaluation, as well as your relationship to that student. Give the specific name of the course that you have the student in or the degree program in which you advise him. Identify the purpose of the evaluation. For example, "This evaluation is for John Starr, student Id XXXXX. John is a student athlete in the Track and Field Program, and the purpose of this letter is to evaluate his grade this semester in Math 1332: Contemporary Mathematics, as requested by the Athletic Department."
Discuss the relevant details requested by the program, such as the student's test grades, homework grades, class participation and attendance. Provide enough detail to create a complete picture of the student's academic progress.
Give an overall assessment of the student's performance. If he is in your class, tell the requestors what grade he has as of today's date. If the student could improve in an area, detail how he could do better and what course of action he needs to take.
Provide any contact information that is not in your letterhead in the final paragraph, and let the recipient know that he can contact you with any additional questions that he has.
Close the letter by typing "Sincerely," skip three line spaces and type your full name. Print the letter on department letterhead and sign in the space above your typed name.
