Gather all of your documents. You need the two-paged verification worksheet, your W-2 tax forms, your parents' W-2 tax forms and any other necessary tax forms. Keep these in a stack so that you do not lose track of them.
Complete your student information on the worksheet. This includes your name, address, social security number, date of birth, and a phone number. Write all of this information with a pen so it easy to read.
Complete your family information. This does not have to include blood relatives, but can include any individuals that your parent(s) support financially. List the name, age, relationship, and college, if applicable.
Check the box that refers to your tax filing status. You may choose to attach a copy of your W-2 and tax returns, or submit these later. Or check the last box if you are not required to file a tax return.
Write in any other sources of untaxed income that includes child support, social security benefits and welfare.
Add in any untaxed earnings that were made the previous year.
Fill in the same tax information for your parent(s).
Double check all of the tax data that you have filled in on the worksheet. You do not want to make an error that could jeopardize your financial aid.
Sign the bottom of the worksheet. Give the worksheet to your parent to sign as well. Add the date in after both signatures.
Make copies of all your tax forms and W-2 tax forms. Sign and date each copy.
Mail or fax the forms to your school’s financial aid office.
