What is the prayer in mezuzah English and Hebrew?

The prayer recited upon affixing a mezuzah to a doorpost is as follows:

In English:

Blessed are You, Lord our G-d, King of the universe, Who has sanctified us by His commandments, and commanded us to affix the mezuzah.

In Hebrew:

בָּרוּךְ אַתָּה ה', אֱלֹהֵינוּ מֶלֶךְ הָעוֹלָם, אֲשֶׁר קִדְּשָׁנוּ בְּמִצְוֹתָיו וְצִוָּנוּ לִקְבּוֹעַ מְזוּזָה.

The mezuzah contains a parchment scroll inscribed with the following verses from the Torah:

Hear, O Israel: the Lord our G-d, the Lord is One. (Deut. 6:4)

And you shall write them upon the doorposts of your house, and upon your gates. (Deut. 6:9)
