There are many types of daycare teacher forms for parents, including those for schedules, illnesses, class trips and tuition, as well as those used in parent handbooks and progress reports. The forms can be bought online from sites such as ADayCare.com (see resources). Some daycares will have a company like Staples or Office Depot print out parent handbooks.
Teachers rely on these forms to let parents know what illnesses are going around or health conditions to be aware of--such as head lice--or to remind them of events and to describe how the child is doing academically. The forms also ensure parental permission for field trips.
Forms identify the responsibilities of the teachers, parents and students, and identify what topics the children need to learn. Teachers should distribute forms to parents that list when papers are due, as well as school arrival and pickup times. Student forms can list subjects in which the child excels, as well as those he needs extra work in.
Ensure that forms are handed out on time. Parent handbooks should be distributed at parent orientations or on the first day of school. Tuition invoices must be sent at least two weeks before payments are due. Parents must be informed immediately of any contagious illnesses that occur in the daycare. Field trip forms should be given to parents a week ahead of time.
Parents need to sign consent forms agreeing with any disciplinary measures used in the classroom. Clearly list infractions and punishments. For example, if a child throws a toy, that could result in a 5-minute timeout in a chair. The list should also include the kind of things that would result in a child getting kicked out of school, such as constant biting or kicking.
